Please download and fill out the following forms and bring to your first appointment. I suggest that you do not email them because email is not a secure form of transmission and could compromise your privacy:

Fees

My fee for a fifty-minute session is $90.
Occasionally, I offer reduced fees for special cirumcumstances.

Insurance

Currently, I do not accept insurance, but will provide a super-bill upon request.
